Your Health in Mount Pleasant, SC is seeking a dedicated Social Worker to join our urgent care family. The role involves assisting patients across facilities and homes in the Charleston area, performing advance care planning, cognitive assessments, placement coordination, and connecting patients with resources.
You must have reliable transportation as travel is required daily. This is a full-time position with 8-hour shifts Monday through Friday (8am-5pm).
