Lily Care Hospice in eastern New Jersey is hiring a full-time Hospice Registered Nurse Case Manager to join our clinical team and help shape hospice care delivery from the ground up. Responsibilities include managing a caseload in homes and facilities, coordinating with physicians and the IDG, educating families, supervising aides, and completing EMR documentation.
The role is based in Florham Park, NJ with a Monday–Friday day schedule and rotating on-call.
