Hospice Registered Nurse Case Manager
Lily Care Hospice is a community-based hospice serving patients and families across eastern New Jersey. We are hiring a full-time Hospice Registered Nurse Case Manager to join our clinical team and help shape hospice care delivery from the ground up.

- Serve as the Hospice Registered Nurse Case Manager for a caseload of hospice patients in private homes and facilities - complete assessments, individualized plans of care, and routine visits
- Provide skilled pain and symptom management in coordination with the attending physician and Hospice Clinical Director
- Participate in IDG meetings and coordinate care across the interdisciplinary team, including hospice aides, social work, and chaplaincy
- Educate and support patients, families, and caregivers, and supervise hospice aide care per the plan of care
- Complete timely, accurate EMR documentation and participate in the RN Case Manager on-call rotation
The Hospice Registered Nurse Case Manager role pays $90,000 - $105,000/year based on experience. Full-time, Monday-Friday day schedule with a rotating on-call assignment. Territory is based out of Florham Park, NJ.
Skills and Qualifications- Current RN license in New Jersey or a compact state
- One year of RN experience required; hospice, home health, or case management experience preferred
- Current CPR certification
- Valid driver's license
