Hospice Community Liaison

Lily Care Hospice is a community-based hospice serving northern New Jersey out of Florham Park. We care for patients where they live — private homes, assisted living, and skilled nursing facilities. We are a growing, nurse-driven organization, not a national chain. Our Hospice Community Liaison owns the territory outright and reports directly to leadership.

We are hiring a Hospice Community Liaison to build our northern New Jersey referral base.

Duties and Responsibilities
  • Build and maintain referral relationships with hospitals, skilled nursing facilities, assisted living communities, and physician practices across the assigned territory
  • Educate referral sources on hospice eligibility, the Medicare hospice benefit, and the Lily Care model of care
  • Develop and execute a quarterly territory plan with defined target accounts and measurable referral goals
  • Plan and deliver in-services and community education events, managing the associated marketing budget
Compensation and Schedule

$85,000 to $100,000 per year, based on experience. The pay range listed represents a general guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate's relevant experience and qualifications. Full-time.

Location:

23 Vreeland Road, Florham Park, New Jersey 07932, with daily travel throughout the assigned northern New Jersey territory. This Hospice Community Liaison role is a field position, not a desk position.

Skills and Qualifications
  • Bachelor's degree
  • Two or more years of hospice or home health business development preffered
  • Established referral relationships in northern New Jersey strongly preferred
  • Valid driver's license, reliable vehicle, and current auto insurance
