Hospice Social Worker
Lily Care Hospice is a community-based hospice serving patients and families across New Jersey. We are hiring a part-time Hospice Social Worker to join our interdisciplinary team and help shape end-of-life care from the ground up. We are looking for someone to join us part-time, with an opportunity to work full‑time as well.

- Serve as the Hospice Social Worker for patients and families on your caseload — assess psychosocial needs and build the social work portion of the individualized plan of care
- Provide counseling and support to patients, families, and caregivers, including anticipatory grief and bereavement follow‑up
- Evaluate insurance coverage and financial need, and connect families to community resources and benefits programs
- Participate in IDG meetings and keep the interdisciplinary team current on psychosocial changes
- Complete timely, accurate EMR documentation and support volunteer and staff orientation
The Hospice Social Worker role pays $36 - $42/hour based on experience. Full‑time, Monday–Friday day schedule. Territory is northern New Jersey, based out of Florham Park, NJ.
Skills and Qualifications- Master of Social Work (MSW) from a program accredited by the Council on Social Work Education
- Current New Jersey LSW or LCSW license issued by the State Board of Social Work Examiners
- One year of social work experience in a healthcare setting; hospice, home health, or geriatric experience preferred
- Valid driver's license and reliable transportation
