Job Summary

As a Hospice Speech Language Pathologist, you will plan, organize, and deliver compassionate, specialized therapy services focused on comfort, communication, and quality of life. Essential Functions

Patient Care

  • Conduct evaluations and assess the home environment to determine equipment and speech/language needs.
  • Identify functional speech and communication deficits; establish an individualized plan of care to improve or maintain essential function.
  • Administer speech therapy programs using specialized techniques and equipment.
  • Recommend communication devices or assistive aides as needed.
  • Instruct patients, families, caregivers, and agency personnel on treatment plans, communication strategies, and safe care techniques.
  • Collaborate closely with referring physicians and the interdisciplinary team to ensure comprehensive, coordinated care.

Communication & Documentation

  • Prepare and submit clinical notes in alignment with organizational policy.
  • Communicate ongoing patient needs, changes, and progress to physicians, caregivers, and team members.
  • Participate in interdisciplinary group (IDG) meetings, in-services, and other required team activities to support holistic care planning.

Additional Responsibilities

  • Maintain clinical competency through continued education and adherence to current SLP practices and theory.
  • Participate in peer consultation, orientation of hospice personnel, and quality performance improvement activities.
  • When appropriate, supervise hospice aides performing tasks related to speech-language therapy.
  • Perform other duties necessary to support effective delivery of SLP services.

Qualifications

  • Master’s degree in Speech-Language Pathology from an approved program, including required internship.
  • Meets education and experience requirements for the Certificate of Clinical Competence (CCC) from ASHA.
  • Certified and licensed to practice Speech-Language Pathology in the state of practice.
  • Current CPR certification.
  • Licensed driver with an insured, reliable automobile meeting organizational requirements.
  • Minimum of one year of clinical SLP experience.
  • Understanding of hospice philosophy and comfort-focused care.
