Your Specific Responsibilities:

This position involves work in Drug Free Hendry, Heart Health Plus, and Healthy Communities under Community Health Promotion. This position is responsible for promoting and coordinating activities to advance health resources and improve health outcomes for residents through partnerships, project planning, and implementation of projects that promote community health and chronic disease prevention.

* Implementation and facilitation of the Heart Health Plus Program (a CDC community-based blood pressure self-monitoring program)

* Provide information and resources regarding substance use through the Drug Free Hendry program

* Collaborate with The Prevention Alliance of Southwest Florida to promote a substance-free community

* Collaborate and engage with community partners in diverse settings (social service agencies, schools, community-based organizations, workplaces, etc.) to implement community health and chronic

disease prevention programs

* Assist with the implementation of the Healthy Communities Program (a CDC program that supports communities with reducing chronic diseases by changing the places and organizations that touch people's lives)

* Research and compile data on community health needs and barriers to achieving health among diverse groups (needs assessments and action plans). This includes information related to the community health needs of Hendry and Glades Counties, barriers to achieving community health, best practices for addressing barriers to achieve health

* Develop and submit program workplans, biannual reports, annual reports, success stories, and other documents as required, within established deadline

* Monitor and evaluate progress on program plan objectives

* Attend outreach and community engagement activities to promote Drug Free Hendry and Heart Health Plus programs

* Assist with preparation of Drug Free Hendry and Heart Health Plus educational materials and promotional items

* Engage and recruit participants for the Heart Health Plus Program

* Attend required trainings and meetings, including technical assistance meetings, monthly state-wide meetings, and quarterly program meetings

* Purchase of office supplies and program materials (receive, organize, reconcile all)

* Assist with program budgets as needed

* Perform other duties as assigned

Required Knowledge, Skills, and Abilities:

* Knowledge of various computer applications, including Microsoft Office programs and online data systems

* Ability to frequently bend, kneel, reach, and sit or stand for long periods of time

* Ability to work independently or with minimal supervision

* Ability to work nights and weekends, as needed

Qualifications:

Minimum:

* Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
