This position is based in Sarasota, FL, serving clinicians in the Manatee/Sarasota/Lee County regions and surrounding areas.

A Nursing Preceptor is a regional role dedicated to providing clinical success in the local branches. A Nurse Preceptor maintains a modified patient caseload and travels to a patient’s home to administer home health services. A Preceptor will participate in co‑visits with employees and assist primarily in the preceptorship of new employees. A Preceptor will also act as an educational mentor to established in‑home field clinicians.

The Regional VP of Operations coordinates assignments on an as‑needed basis.

• Hosting Local Clinical Orientation
• Competency and Skill Sign‑Off for Nurses
• Travel to all branches in the region for support
• Conducting in‑home OASIS assessments, evaluations, and developing personalized, goal‑driven treatment plans
• Providing clinical oversight and guidance to field clinicians
• Collaborating with patients, caregivers, and a supportive clinical team to deliver exceptional care
• Utilizing dictation services to streamline documentation and maximize time for patient care
• Delivering hands‑on nursing care, including:
  • Acute & chronic disease management
  • Medication management
  • Post‑surgical rehabilitation support
  • Pain management
  • Wound care

• Unincumbered Florida Registered Nurse license (required)
• Valid driver’s license, auto insurance, reliable transportation (required)
• Two years of Home health care experience (required)
• Medicare OASIS documentation proficiency (required)
• Kinnser/Well Sky EMR system (required)
• Ability to travel daily between Clay, Flagler, and occasionally Volusia Counties, and surrounding areas (required)
