DescriptionIf you’re an Occupational Therapist (OT) who wants to build meaningful relationships with your patients and see the long-term impact of your care—this is it.
In this setting, you will work with Medicare Part B patients in Assisted Living and Independent Communities, allowing you to follow patient progress over time and focus on improving independence, mobility and quality of life.
Unlike fast-paced short-stay environments, this role allows you to provide purposeful, patient-centered therapy, with consistency and clinical autonomy.

- Treat Medicare Part B patients in Assisted Living & Independent Living Communities

- Evaluate and treat residents in Assisted Living & Independent Living Communities
- Develop personalized care plans focused on functional independence, safety, and quality of life
- Deliver care through therapeutic activities, ADL/IDL training, and adaptive techniques
- Help patients improve their ability to perform daily tasks such as dressing, bathing, grooming, and meal preparation
- Collaborate with care teams, staff, and caregivers to achieve strong patient outcomes
- Document patient progress and adjust treatment plans as needed
- Licensed Occupational Therapist (OT) or license eligible in the state of practice
- Graduate of an accredited Occupational Therapy Program
- Interest or experience in geriatric rehabilitation or senior living therapy
- Passion for patient-centered care and improving quality of life for older adults
