• Evaluate the psychosocial aspects of patients and families and assist in the referral process to community agencies.

• Florida driver’s license required within 30 days of hire.
• Master’s degree in Social Work or Counseling.
• 1 year of Social Work experience.
• Preferred 1 year of Home Care experience.
