Job Summary
Works with enrolled parents to assist them in creating and building connections in order to assist them to become self‑sufficient. Provides case management and outreach services. Assesses clients to identify their family strengths, challenges, and needs. Collaborates with community partners to build connections. Assists and facilitates parent‑led support groups and parent leadership meetings. Helps to engage parents in Community Action Network meetings.
Essential Duties & Responsibilities- Demonstrate cultural awareness and sensitivity when working with families from diverse backgrounds.
- Create and build connections with persons served to assist them in meeting their needs in order to become self‑sufficient.
- Provide specialized case management and outreach services to individuals with perinatal mental health issues and substance use disorders.
- Assume responsibility for assessing persons served and family strengths, challenges, and needs.
- Collaborate and build connections with community partners.
- Assist and facilitate parent‑led support groups and parent leadership meetings.
- Help engage parents through lived experience and support them in contributing to the Community Action Network.
- Attend meetings promptly and actively participate in trainings, staff meetings, and clinical supervision.
- Complete documentation in a timely and accurate manner.
- Complete all other tasks assigned by Program Manager within given time frames.
- Remain familiar with and utilize community and formal resources, accessing informal resources first before formal ones.
- Effectively communicate via written, verbal, in‑person, and virtual methods.
- Work efficiently in a fast‑paced environment.
- Maintain strong time‑management and multitasking skills to handle a variety of duties concurrently.
- Demonstrate strong interpersonal skills and attention to detail when speaking with and collecting information from clients and referral sources.
- Operate Microsoft Word, Microsoft Excel, and electronic health record systems.
- Education Level: High school diploma or GED required. Bilingual preferred.
