JOB OVERVIEW
This position performs health education work for the Healthy Start Program and the Okaloosa County Health Department (OCHD). Work is performed under the supervision of the Healthy Start Supervisor and the direction of the Healthy Start Program Director, with strict adherence to all Florida Department of Health (FDOH), OCHD and Healthy Start policies and guidelines.
RESPONSIBILITIES- Conduct case management and provide care coordination for Healthy Start clients of Okaloosa County, including women during pregnancy and their infants after delivery.
- Provide health education and care coordination to clients and families in a variety of settings and modalities, such as phone, video conferencing, in‑office/clinic, and at clients’ homes, workplaces, and schools.
- Record all applicable work‑related mileage, maintain travel approval/reimbursement documentation, and adhere to travel‑related policies (DOHP 56‑37‑22 Travel Policy).
- Educate clients and families on prenatal health, parenting skills, infant/child health, health education resources, and referrals to other providers.
- Facilitate psychosocial assessments, ASQs, Domestic Violence Screenings, and appropriate referrals for high‑risk clients.
- Develop individual client and family support plans to ensure appropriate services and minimize obstacles to prenatal and infant health care.
- Document all client activities and code services in the Well Families System accurately and on time.
- Assist in collecting, summarizing, and evaluating data for program improvement.
- Participate in monitoring, QI‑QA activities, and organizational education/outreach activities.
- Attend staff meetings, conferences, training sessions, committees, and work groups as requested.
- Meet with Supervisor or Program Director to review performance, receive coaching, and report concerns promptly.
- Perform additional duties as assigned in support of the Agency’s mission and vision.
- Knowledge of prenatal and maternal health and general health education practices.
- Ability to conduct group meetings and plan/implement training activities.
- Effective communication and relationship‑building skills.
- Physical ability to lift and carry up to 25 pounds.
- Ability to provide childbirth education, breastfeeding support, smoking cessation guidance, and parenting instruction.
- Proficiency with computer equipment and Microsoft Office applications.
- Access to a personal vehicle for home‑based or off‑site client visits; mileage reimbursement per DOHP 56‑37‑22 Travel Policy.
- Valid U.S. driver’s license.
- High school diploma or equivalent (per contract between OCHD and Healthy Start Coalition).
- Competitive salary ($42,561.48 – $43,272.32 annually).
