About the Role

As a Care Coordinator, you’ll independently assess needs, coordinate services, and advocate for adults with opioid use disorders receiving Medication Assisted Treatment (MAT). You’ll help ensure clients receive comprehensive, coordinated care while maintaining continuity of services.

Key Responsibilities
  • Conduct comprehensive assessments and confirm treatment needs
  • Develop and implement individualized service plans
  • Coordinate care with providers and community agencies
  • Advocate for clients and connect them to resources
  • Monitor progress and maintain accurate documentation
Qualifications
  • Bachelor’s degree in healthcare, psychology, social work, or related field
  • 1+ year of care management experience (SUD, SMI, or similar populations)
  • Valid Florida driver’s license with clean record
