  8:00 AM - 5:00 PM, Monday-Friday, 1 weekend rotation per month
• Weekend Work: Occasional
• On Call: Yes
• Home visits in assigned service area: Sarasota and Charlotte County

• Assists Physical Therapist with the administration of interventions utilized in physical therapy and rehabilitation
• May progress patients in the treatment plan developed by the physical therapist
• Communicates with all physicians involved in the plan of care and other healthcare practitioners (as appropriate) related to the current plan of care
• Follows established procedures, emergency and safety precautions at all times in the treatment of patients and use of modalities
• Maintains timely communications, records, reports, and other documentation in accordance with agency policies & procedures, and Medicare guidelines
• Captures documentation during the home visit following company policy using company issued laptop
• Assists in progressing therapy programs with prior assessments and approval, prepares clinical progress notes and participates in case conferences and utilization reviews

• Physical Therapist Assistant State of Florida licensure
• Driver’s License State of Florida (Florida driver's license is required within 30 days of hire)
• Basic Life Support

• Associate's Degree in Physical Therapy Assistant

• 1 year Physical Therapist Assistant experience
• Preferred 1 year Home Care experience
