Overview

Coordinating residential and community services for clients in the Forensic Residential Out-Patient. Communication with the Court and all parties regarding the clients placed on a conditional release by the respective Circuit Courts at the direction of the Program Supervisor. Providing case management services, including assessing clients' needs, conducting psychosocial evaluations, developing individual treatment plans, referral and linking, providing client transportation, monitoring all aspects of care and treatment, and maintaining documentation of services and staff activity.

Providing education and support to clients and family members. Assuring intra-agency (Apalachee) and inter-agency (community) continuity of care for assigned caseload. Preparing and maintaining client records in accordance with all applicable regulations and standards.

ESSENTIAL/CORE JOB-SPECIFIC DUTIES AND RESPONSIBILITIES
  • Provides Case Management services for assigned clients to include assessment, planning, linking, monitoring and advocacy in accordance with Agency and court policies and procedures and established clinical protocols. Ensures activities are aimed at linking the service system to the client and coordinating the various system components to achieve a successful outcome.
  • Completes required Progress Notes and clinical documentation for all service activities and maintains clinical records in accordance with Client Record Handbook specifications and Agency policies and procedures.
  • Meets requirements of 90% of CSP/SPR completed as required per policy manual.
  • Meets Service Productivity standard of at least 75% (or 6 out of 8 work hours) for the provision of Case Management services in accordance with applicable clinical/quality criteria which are supported by appropriate clinical record documentation which meet Client Record Handbook specifications, SAL documentation requirements, and Agency business/clinical ethics standards.
  • Completes progress reports to the probation court on a monthly basis or as specified in the conditional release regarding the resident's progress in treatment and compliance with the program.
  • Tracks, attends and coordinates transportation to court hearings. If hearings are out of our catchment area, the attendance will be determined with the Program Supervisor on a case by case basis.
Qualifications

Certified Behavioral Health Case Manager (CBHCM) verified through the Florida Certification Board (FCB) in accordance with Florida Statute 394.4573(e). OR Bachelor’s Degree in counseling;
Social Work;
Psychology;
Criminal Justice;
Nursing;
Rehabilitation;
Special Education;
Health Education; or a Human Services Field in which major coursework includes the study of human behavior and development;
Must obtain Certified Case Manager – obtain Provisional Status (CBHCM-P) within 3 months of hire. Once provisional status is obtained, must obtain Certified Behavioral Health Case Manager (CBHCM) within 12 months. If possess the Provisional status upon hire, must obtain the full certification of CBHCM within 12 months of employment.

Licensure or Certification
  • Must maintain current ADM Targeted Case Manager Certification.
  • Must possess current, valid driver's license and maintain Apalachee Designated Driver status.
