Job Summary:

Our client is seeking highly motivated and compassionate Home Health PT Physical Therapists fulltime for our [insert cities/county] Team. As part of our Physical Therapy Team, you will be part of a dynamic Clinical Team who ensure that our patients return and maintain their optimum level of physical functioning. Job Responsibilities:

• Provide physical therapy services to patients as prescribed by a physician in compliance with agency policies and procedures and federal and state regulations.
• Perform initial and ongoing assessments, including OASIS assessments at appropriate time points, to determine level of functioning and patient's ability to perform activities of daily living.
• Develop and revise plans of care, in consultation with the physician and other care team members, which will enable patients to achieve realistic goals of mobility.
• Reports to the physician and designated agency representative patient's reaction to treatment and any changes in the patient's condition.

Required

Skills & Experience:

• One to two (1-2) years of experience.
• Minimum of one (1) year of experience in home health care.

Required

Education:

• DPT from CAPTE accredited program.
