Ops Senior Human Services Program Special
This position coordinates care between numerous healthcare providers; refers patients to providers who can assist them. Shares health and wellness tips relevant to patients. Advocates for patients attempting to make use of public health and government programs. Explains health information in a culturally sensitive, easy-to-understand manner. Advises clients/community groups on issues related to improving general health, such as diet or exercise;
leads support groups, workshops and seminars on topics of interest to the community. Creates health events for the Florida Department of Health to promote wellness within Nassau County; attends community outreaches to distribute flyers, brochures, and other informational or educational documents to perform outreach and awareness of the Department of Health programs. Maintains accurate records of all patient contacts; protects patient confidentiality.
Develops constructive and cooperative working relationships with others. Conducts home visits for targeted population clients to monitor their progress or assess their needs for resources in the paramedicine program.
Competence in Microsoft Office and assorted software. Competence in the use of an EMR for "real time" documentation. Competent in the use of equipment and supplies related to the position. Ability to provide Patient/Family centered care and resources. Ability to plan, organize and coordinate work assignments. Ability to communicate effectively. Ability to establish and maintain effective working relationships with others.
Minimum – High School Diploma. Valid Driver's License. Must be able to bend, squat, or stand for prolonged time, and lift items up to 50 pounds. Participate in disaster preparedness and response activities as assigned. These activities may require working before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the employee includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
Preferred – Bilingual in Spanish and English. Community Health Worker certification. Healthcare certifications/licenses or background experience.
