Location: Macclenny
The Senior Human Services Program Specialist serves as a Discharge Coordinator at Northeast Florida State Hospital, a 613-bed mental health treatment facility. The role involves planning and coordinating discharge activities for individuals with severe mental illness, collaborating with community providers, and ensuring successful transitions to community-based care. The position requires independent judgment, strong organizational skills, and the ability to work across hospital and community partners to address barriers to discharge.
Key qualifications:
- Knowledge of social, economic, rehabilitation or health care service objectives
- Ability to develop and monitor policies, procedures, and program standards
- Ability to communicate effectively and establish liaison with other agencies
- Skill in compiling, organizing and analyzing data
- Ability to plan, organize and coordinate work assignments
- Strong attention to detail and independent judgment
