This role is primarily onsite, engaging with patients in their residential settings, allowing you to experience the unique dynamics of home therapy. You will work a standard Monday to Friday schedule, collaborating closely with fellow healthcare professionals and nurturing a holistic approach to patient recovery. • Conduct thorough evaluations, including patient history assessments and relevant clinical tests, to form the foundation for personalized care plans. These evaluations are meticulously documented to provide a comprehensive overview of each patient's condition.
• Communicate your evaluation findings clearly to patients and their families, actively involving them in setting achievable treatment goals tailored to their individual needs.
• Demonstrate clinical expertise by implementing treatment protocols based on the latest evidence, adapting your approach as needed to respond to patient progress and changes in their health status.
• Engage in collaborative discussions with other healthcare professionals as necessary, identifying changes in patients' conditions that might necessitate adjustments to their care plans.

Your primary focus will be a diverse patient population requiring specialized physical therapy in the home care setting. • A Doctorate degree in Physical Therapy (DPT) is essential to register for this role.
• Must hold a valid Physical Therapy license in the State of Florida; licensing must align with the specific geographical requirements in home care.
• All candidates are required to have certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.

While a minimum of 2 years of experience in physical therapy is required, candidates with a proven background in home care settings and outpatient therapy environments are highly preferred. Experience in managing diverse patient demographics and conditions will be advantageous in providing superior care.
