Provides high-quality direct services at the center(s) in the areas of health, disability, safety, and nutrition. Coordinates and carries out direct comprehensive health services for children and families that include a range of medical, dental, and nutritional services and follow-up in accordance with goals and federal performance standards. Works hands-on with children and families and therefore must be able to communicate and respond in an effective manner that consistently demonstrates respect and concern.

• Arranges and conducts the hearing, vision, and dental screenings on all enrolled children; or coordinates with local resources to conduct these screenings, assisting in their screening process as needed.
  Follow-ups after the screenings are done in collaboration with the center team
  :
  1) Shares all results with families.
  2) Ensures children needing dental exams and follow-ups treatment or vision/hearing get services that are documented in the child’s file/electronic record.
  3) Complete and provide families with referrals and consistent follow-ups. Follows the standards and time frames listed in the RCMA Plan of Action.
• Ensures,
  in collaboration with center team
  , that all children stay current on:
  • Immunization status, and utilize information available through Florida SHOTS.
  • Well Child Check-ups following the Florida Medicaid (EPSDT) periodicity schedule.
  • All other health screenings are completed and documented (blood lead and hemoglobin/hematocrit).
  • Reviews the Health & Nutrition Assessment from My Head Start  and provides families with referrals and consistent follow-up.
• Assists the center team, when necessary, in making referrals for disability services to Early Steps, FDLRS, or other agencies. Coordinates the teamwork necessary to complete the disabilities summary sheet and reviews it for accuracy prior to submitting it to the Health, Safety, and Nutrition Specialist on a monthly basis. Works in collaboration with families and the family’s FSW to arrange services including providing or arranging transportation as needed for any health and/or disability-related appointment.
