Concierge Home Care is hiring a Home Health Physical Therapist (PT). If you’re looking for more control over your schedule, meaningful patient relationships, and the ability to increase your income based on productivity—this is the role for you.
Coverage Area:Tallahassee, FL / Leon, Franklin or Taylor Counties
Territories designed to minimize drive time with clustered scheduling
- Salary+ model: $110,000 – $117,000+ for 30 productivity points
- Pay-per-visit at $70+ per visit + Medicare differential pay for all visits over 30 points

- Complete documentation using streamlined technology
- Health insurance (starts after 30 days)

- Active, unencumbered Florida Physical Therapist license
- Valid driver’s license, reliable transportation, and auto insurance
