Position Summary
The Annual Wellness Visit (AWV) LPN is responsible for independently conducting the patient‑facing components of Medicare Annual Wellness Visits. This role focuses on guiding patients through required questionnaires, screenings, and health discussions, ensuring all visit elements are completed accurately and efficiently prior to the provider’s portion of the visit. The LPN works closely with providers and clinical staff to support high‑quality, compliant wellness visits.
Key Responsibilities- Conduct Medicare Annual Wellness Visits by completing all required questionnaires, screenings, and assessments (e.g., health risk assessment, depression screening, fall risk, functional status, and preventive care needs).
- Guide patient conversations in a professional, clear, and compassionate manner to obtain accurate information.
- Work independently to manage the AWV workflow from patient scheduling through visit completion.
- Schedule patients for Annual Wellness Visits and coordinate follow‑up care or appointments as needed.
- Identify patient needs, health risks, and gaps in care and communicate findings to the provider.
- Ensure all documentation is complete, accurate, and compliant with Medicare requirements.
- Collaborate directly with providers to transition care smoothly for the provider’s portion of the visit.
- Maintain a patient‑centered approach while supporting practice goals and quality measures.
Full‑time Monday to Friday, ~8 AM to ~5 PM and on‑site at 6600 University Pkwy., Suite 201, Lakewood Ranch, FL (3 days per week) and 14616 State Road 70, East Lakewood Ranch, FL (2 days per week).

- Education
:
Completion of LPN/LVN nursing training from an accredited school of nursing;
Minimum of one (1) year college or Technical School;
Associates Degree preferred. - Active Licensed Practical Nurse (LPN) license in good standing.
- Current BLS/CPR certification required.
- Current state registered license for LPN/LVN.
- Work experience
:
Minimum of one (1) year working as a Licensed Practical/Vocational Nurse required.
- Knowledge
:
General medical office procedures. Knowledge of Medicare Annual Wellness Visit requirements preferred (training may be provided).
- Skills
:
Proficiency in basic computer programs and operating systems, such as Microsoft Office. Basic computer and electronic medical record (EMR) proficiency. Excellent communication, organizational and interpersonal skills. Strong communication and interpersonal skills. Bilingual in English and Spanish preferred. Ability to work independently and manage multiple tasks. Comfortable engaging patients in health‑related conversations.
