Seeking a compassionate Hospice RN Case Manager to provide patient-centered end-of-life care in home and community settings. This role manages a caseload, coordinates interdisciplinary care, and supports patients and families through every stage of the hospice journey.

• Perform comprehensive patient assessments
• Develop and manage individualized care plans
• Administer medications and treatments per hospice protocol
• Coordinate care with physicians and interdisciplinary team
• Educate families on symptom management and disease progression
• Document accurately in EMR systems

• Active Florida RN license
• 2+ years of nursing experience (required)
• Hospice or palliative experience preferred
• Day shift availability required
• Comfortable driving up to 40 miles
