Responsibilities include:
- Admissions, Discharge and Transfers of residents
- Medication requisitions with pharmacy and physicians
- Medication administration
- Follow infection control, emergencies, and other center policies
- Collaborate with resident’s interdisciplinary team to provide best outcomes
- Identify and report all abnormal changes in residents’ condition to proper personnel per center policy
- Work under direct supervision in accordance with the state‑specific Nursing Practice Act, facility policies and procedures, and nursing judgment
- Deliver nursing care to patients/residents requiring long‑term or rehabilitative care
- Collect patient/resident data, make observations, and report pertinent information related to the care of the patient/resident
- According to state‑specific regulations, implement the patient/resident plan of care and evaluate the patient/resident response
- In accordance with state‑specific regulations, direct and supervise caregiving by other nursing personnel in selected situations
- Maintain knowledge of necessary documentation requirements
- Maintain knowledge of equipment set‑up, maintenance and use (e.g., monitors, infusion devices, drain devices, etc.)
- Maintain confidentiality and patient/resident rights regarding all patient/resident/ personnel information
Qualifications:
- LPN (Licensed Practical Nurse) licensed by the state
- CPR Certification required
- Two years prior nursing experience preferred
