The Annual Wellness Visit (AWV) Licensed Practical Nurse (LPN) is responsible for independently managing the patient‑facing components of Medicare Annual Wellness Visits. This role involves guiding patients through necessary questionnaires, screenings, and health discussions to ensure all aspects of the visit are completed accurately and efficiently before the provider takes over. The LPN collaborates closely with providers and clinical staff to deliver high‑quality, compliant wellness visits.
- Conduct Medicare Annual Wellness Visits by administering all required questionnaires, screenings, and assessments, including health risk assessments, depression screenings, fall risk evaluations, functional status reviews, and preventive care needs.
- Lead patient interactions in a professional, clear, and compassionate manner to gather accurate information.
- Independently oversee the AWV process from patient scheduling to the completion of the visit.
- Schedule patients for Annual Wellness Visits and arrange any necessary follow‑up care or additional appointments.
- Identify patient health needs, risks, and care gaps, then communicate these findings to the provider.
- Ensure all visit documentation is thorough, precise, and meets Medicare compliance standards.
- Work directly with providers to ensure a seamless transition for the provider‑led portion of the visit.
- Maintain a patient‑focused approach while supporting the practice’s quality measures and goals.
- Completion of LPN/LVN nursing training from an accredited school of nursing;
Minimum of one (1) year college or Technical School;
Associate’s Degree preferred. - Active Licensed Practical Nurse (LPN) license in good standing.
- Current BLS/CPR certification required.
- Current state registered license for LPN/LVN.
- Minimum of one (1) year working as a Licensed Practical/Vocational Nurse required.
- General medical office procedures.
- Knowledge of Medicare Annual Wellness Visit requirements preferred (training may be provided).
- Proficiency in basic computer programs and operating systems, such as Microsoft Office.
- Basic computer and electronic medical record (EMR) proficiency.
- Excellent communication, organizational and interpersonal skills.
- Strong communication and interpersonal skills.
- Bilingual in English and Spanish preferred.
- Ability to work independently and manage multiple tasks.
- Comfortable engaging patients in health‑related conversations.
- Must be able to perform courteously and professionally in all interactions.
- Office equipment such as fax machine, copier, and telephone system.
- EKG machine.
- Computer.
- Works in a well‑lighted, OSHA Standard environment.
- May be exposed to infectious and contagious diseases.
- May be in contact with patients under a wide variety of circumstances.
- Occasionally subject to irregular hours.
- Travel to other facilities as requested.
- May be required to wear personal protective equipment as necessary.
- Must be able to handle emergency or crisis situations.
- Must be able to see with corrective eye wear.
- Must be able to hear clearly with assistance.
