We are seeking a skilled and compassionate
Nurse Practitioner (NP)to join our Cardiology team in a
Regional Flex Provider role
. The ideal candidate will have experience in cardiovascular health and be comfortable managing patients with a variety of cardiac conditions in
skilled nursing and post-acute care settings
.
This role is designed for a flexible and adaptable provider who enjoys working across multiple facilities and supporting clinical operations where needed. The Regional Flex Provider will travel within a 90 mile radius of their home base
to support our partner facilities.
Responsibilities will include
providing coverage for provider PTO or scheduling gaps, assisting with onboarding of new facilities, supporting facilities that require additional clinical coverage, and helping implement education and cardiology care initiatives.
This is a full time position with flexible hours. The candidate will be expected to work5 days per week
, with compensation ranging from$130,000 – $160,000 annually
with bonus incentives. Some weekends as necessary. Travel within a 90 mile radius
will be required.
Conduct comprehensive cardiovascular assessments, including patient history and physical exams
Diagnose and manage acute and chronic cardiovascular conditions
Provide cardiology care to patients in skilled nursing and post-acute care facilities
Travel to facilities within a 90 mile radius to provide clinical coverage and operational support
Provide temporary coverage for provider PTO, scheduling gaps, or increased patient demand
Support facilities requiring additional clinical oversight or cardiology support
Order and interpret diagnostic tests such as EKGs, echocardiograms, stress tests, and lab work
Develop and implement treatment plans in collaboration with cardiologists
Provide patient education on heart health, disease prevention, medications, and lifestyle modifications
Monitor and manage patients with hypertension, heart failure, atrial fibrillation, and other cardiac conditions
Perform follow-up visits and manage chronic disease care
Provide education and support to facility staff regarding cardiovascular care best practices
Maintain accurate and timely electronic health records (EHR)
Collaborate with a multidisciplinary team to ensure the highest quality of patient care
Qualifications:Board Certification Nurse Practitioner
Valid state license and certification as a Nurse Practitioner
2 years Experience in
cardiology and cardiovascular care highly preferred
Experience working in
skilled nursing facilities, long-term care, or post-acute care settings
Ability and willingness to
travel within a 90 mile regional territory
Strong clinical assessment skills and the ability to interpret diagnostic tests
Excellent communication skills, both verbal and written, with a focus on patient education
Ability to work effectively within a team-oriented environment
Proficiency in electronic health record (EHR) systems
Current ACLS Certificate
Current BLS Certificate
