Job Overview

Licensed Practical Nurse (LPN) – Oak Trace Community, 200 Village Drive, Downers Grove, Illinois 60516.

Hourly pay range: $30.88–$42.43. Starting rate: $34+ (depending on experience). PRN Bonus Program, flexible schedules, up to $8,000 in scholarship and tuition assistance, career development and promotional opportunities, 5‑STAR community with outstanding staffing ratios.

• Participate with interdisciplinary team to review, plan, coordinate and evaluate resident care.
• Document resident condition and nursing needs accurately and in a timely manner.
• Report pertinent observations and reactions regarding residents promptly.
• Develop, direct, and monitor nursing assistant assignments; adjust assignments based on census and level of care required.
• Execute treatments as necessary and document reactions to medications and treatments.
• Generate, prepare, administer, and chart all medications.
• Initiate physician orders.
• Facilitate communication with families regarding changes in medications and/or resident status.
• Establish protocol to ensure care plans are continually reviewed and updated with accurate data representing each resident's individuality.
• Coordinate admissions, discharges, and transfers to deliver quality customer service.

• Licensed Practical Nurse graduate from an accredited nursing program.
• Holder of a current Licensed Practical Nurse license and any additional licensure required by state regulations.
