• Provide high‑quality, patient‑centered care to residents in skilled nursing facilities.
• Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues.
• Communicate with facility nursing staff via phone or secure messaging as needed.
• Maintain reliable on‑site attendance for scheduled shifts.
• Ensure compliance with clinical standards, facility policies, and regulatory requirements.

• Advanced Practice Providers must hold an active U.S. license and be board‑certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants).
• Qualified specialties include, but are not limited to, internal medicine, family medicine, emergency medicine, general practice, physical medicine and rehabilitation, and geriatric medicine.
• Employees must be able to perform the essential functions of this position satisfactorily, with or without a reasonable accommodation.
