Aveanna Healthcare, a leading provider of home healthcare services, is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide private duty nursing care to patients in their homes. This role is pivotal in meeting the diverse healthcare needs of patients ranging from pediatric to elderly populations across the United States, ensuring their comfort and enhancing their quality of life through tailored nursing services.

The ideal candidate will possess a strong dedication to patient care, with the ability to work closely with families and other healthcare professionals to implement effective care plans.

Responsibilities
  • Administer medications, manage pain relief, and monitor vital signs as prescribed by a physician.
  • Regularly assess the patient's condition and report any changes to the physician and relevant healthcare team.
  • Provide patient and family-centered care by educating them on the management of their condition.
  • Maintain accurate and detailed patient medical records and report any inconsistencies.
  • Work collaboratively with a multidisciplinary healthcare team to implement and adjust individualized care plans.
  • Ensure a safe environment for the patient at home, adhering strictly to professional standards and safety regulations.
  • Proficient in providing patient-centered nursing care.
  • Able to operate medical equipment.
  • Strong organizational and communication skills.
  • Capability to maintain composure in stressful situations.
  • Keen observation skills for monitoring patient health.
Qualifications
  • Valid LPN license in the state of practice.
  • CPR certification.
  • Strong organizational and communication skills.
  • Proficient in providing patient-centered nursing care.
