Role Overview
The RN, Case Manager is responsible for assessing and identifying patient/family needs, utilizing the nursing process, coordinating the Plan of Care with the Interdisciplinary Team (IDT), and providing clinical, palliative and supportive care to the patient/family unit in order to keep the participant in their home environment as long as possible.
Responsibilities- Provide and manage direct care to patients and families as part of the IDT, incorporating psychosocial, spiritual, cultural, physical and biological components.
- Coordinate the Plan of Care, ensuring an individualized plan reflects the patient’s evolving needs.
- Educate patient, family, caregivers and other health professionals about disease process, prevention, palliative interventions, caregiving, dying process, and safety practices.
- Perform home visits to assess safety, medication compliance, nutritional compliance, and durable medical equipment compliance.
- Report changes in the patient’s condition to appropriate members of the IDT or other health professionals.
- Participate with the IDT to evaluate hospice referrals/admissions for level‑of‑care appropriateness and attend daily IDT collaboration meetings.
- Present concise and pertinent oral and written reports to the IDT; encourage input from all disciplines.
- Communicate accurately and completely to physicians, staff members, patients, families, and supervisors; utilize positive approaches when working with others.
- Supervise patient care provided by Community Health Workers and Home Health Aides as requested.
- Respond to emergencies (e.g., hurricanes) by reporting to designated locations and providing continuity of services.
- Perform other duties as assigned.
- Current RN license in the state where the employee will be working.
- Minimum of one year of nursing experience; hospice or hospital experience preferred.
- Certification of completion of Alzheimer’s Disease and Related Dementias Training through the Florida Department of Elder Affairs (for PACE employees).
- Prior experience with an EMR/EHR system.
- Valid driver’s license and automobile insurance per company policy; reliable transportation.
- Ability to use equipment with visual and auditory mechanisms.
- Effective verbal and written communication in English.
- Ability to perform essential functions and physical requirements, including lifting patients and equipment, bending, pushing, pulling, and kneeling.
- Active Basic Life Support (BLS) for healthcare professionals from the American Heart Association or Red Cross.
- Provides reassurance on the phone to patients and families, assists in finding solutions, and coordinates in‑person visits when needed.
- Utilizes appropriate support/expert resources to resolve complex situations.
- Documents patient/family contact information in the EMR and communicates with the IDT.
- Completes initial and semi‑annual assessments for all services.
- Obtains physician orders and completes documentation per company policy.
- Acts as the company representative at assigned facilities, facilitating referrals to all service lines.
- Communicates frequently with IDT members and provides clinical communication in a timely manner using SBAR.
The role requires the ability to see, hear, talk, walk, use hands, stand, sit, reach, lift, bend, balance, pull, push, stoop/crouch, kneel, and climb stairs.
Compensation and Employment Conditions- Pay Range: $61,860.66 – $96,657.28.
- Requires consent to drug and/or alcohol testing after a conditional offer of employment and ongoing compliance with the Drug‑Free Workplace Policy.
- All employees performing services for Florida affiliates undergo background screening through the Florida Care Provider Background Screening Clearinghouse after a conditional offer of employment and ongoing eligibility checks.
Monday – Friday, 8:00 a.m. – 5:00 p.m. (no weekend work).
