Location: Sarasota
Concierge Home Care | Sarasota / Sarasota County, FL 34238 Position
Part‑Time RN – Evening & Weekend Coverage, Flexible Schedule
Compensation- $48–$50 base visit rate
- $10 differential for evening/weekend visits
- $225 weekly on‑call stipend
- Minimum productivity expectation: 18 points per week
- Mileage reimbursement (per company policy)
- Supplemental income with predictable on‑call compensation
- Complete skilled nursing visits in the evenings and weekends
- Perform Start‑of‑Care (SOC) assessments, Resumption of Care visits, and PRN visits
- Respond to after‑hours patient calls and triage concerns appropriately
- Coordinate care with physicians, case managers, and interdisciplinary care teams
- Provide comprehensive patient assessments, medication reconciliation, wound care, disease‑management education, and catheter care
- Support urology‑related patient needs:
Foley catheter management, nephrostomy care, urinary retention monitoring, post‑surgical urology follow‑up, and infection prevention education - Complete documentation within required EMR time frames
- Participate in rotating evening/weekend on‑call coverage
- Support late‑add visits, telephone triage, and Start‑of‑Care admissions
- On‑call availability: 4:00 PM – 8:00 AM (Monday–Friday)
- Weekend coverage (rotating):
- Saturday 8:00 AM → Monday 8:00 AM
- Saturday 8:00 AM–8:00 PM, Sunday 8:00 AM–8:00 PM, overnight triage unless an emergent visit is required
- Minimum weekly productivity: 18 points
- Point opportunities:
Start‑of‑Care admissions, skilled nursing revisits, PRN urgent visits, Resumption of Care visits, weekend precepting support
- Active Florida RN license or Compact RN license
- Experience in home health, urology, med‑surg, hospice, or acute care preferred
- Strong assessment and triage skills
- Experience with OASIS documentation preferred
- Ability to work independently
- Reliable transportation, valid driver’s license, and auto insurance
- Preference for flexible evening work
- Comfortable managing urgent patient needs
- Strong Foley/urology management experience
- Seeking supplemental income with predictable compensation
