As an RN Case Manager, you'll coordinate and oversee the interdisciplinary plan of care for an assigned caseload of hospice patients, ensuring they receive compassionate, high-quality care wherever they call home. Responsibilities include developing individualized care plans, managing symptoms, educating patients and families, collaborating with physicians and the interdisciplinary team, supervising Hospice Aides and LPNs, completing timely documentation, and serving as the clinical advocate throughout the patient's hospice journey.
Key Responsibilities- Manage an assigned caseload of hospice patients
- Complete comprehensive patient assessments and ongoing evaluations
- Develop, implement, and update individualized care plans
- Coordinate care with physicians and the interdisciplinary team
- Provide education and emotional support to patients and families
- Supervise Hospice Aides and Licensed Practical Nurses
- Maintain accurate and timely electronic medical record documentation
- Participate in quality improvement initiatives and interdisciplinary meetings
- Deliver exceptional, compassionate end-of-life care while upholding Big Bend Hospice's mission and values
- Current Florida Registered Nurse (RN) license
- Minimum of one year of inpatient nursing experience or previous hospice, home health, or long-term care experience
- Excellent communication and relationship-building skills
- Strong clinical assessment and critical thinking abilities
- Valid Florida driver's license, reliable transportation, and current auto insurance
- A passion for serving patients and families with compassion and professionalism
