Assistant Director of Health & Wellness Position Summary
Responsible for the planning, implementation, and evaluation of all resident care services to ensure the highest quality of care within the community.
The responsibilities listed below are representative of the role and are not intended to be all-inclusive. Additional duties may be assigned as needed.
- Deliver care in accordance with all state and federal regulations, as well as community policies and procedures
- Participate in pre‑admission assessments for prospective residents of the community
- Complete resident assessments upon admission and conduct ongoing evaluations of physical, functional, and cognitive status
- Develop, review, and update care plans based on resident needs and regulatory requirements
- Administer medications and treatments in accordance with physician orders and the five rights of medication administration
- Monitor residents for changes in condition and communicate updates to physicians, families, and community leadership as appropriate
- Assist with scheduling medical, dental, and laboratory appointments
- Support physicians during examinations as needed and accurately transcribe physician orders
- Supervise residents who self‑administer medications and monitor restorative nursing programs
- Assist residents with daily living needs, including nutrition and hydration, and document appropriately
- Monitor mealtimes, assist with serving trays, and support residents with feeding when needed
- Maintain a safe, clean, and welcoming community environment for residents
- Respond promptly to resident call lights and emergency situations, including Independent Living calls per community policy
- Follow all fire safety and emergency preparedness procedures during drills and actual events
- Promote positive relationships with residents, families, visitors, and team members
- Maintain confidentiality of all resident health information
- Participate in community quality assurance and risk management programs
- Support staff education and training initiatives
- Maintain reliable attendance and punctuality
- Reports directly to the Director of Health & Wellness
- Provides leadership to community team members, including hiring, training, scheduling, and performance management
- Conducts performance evaluations and supports disciplinary actions when necessary
Education & Experience:
- High school diploma or GED required
- Active LPN license in the applicable state required
- Minimum of 5 years of progressive experience in long‑term care, senior living, or a related healthcare setting
- Equivalent combination of education and experience may be considered
- Strong understanding of nursing practices, regulations, and standards within senior living communities
- Ability to communicate clearly and effectively, both verbally and in writing
- Demonstrated ability to lead and motivate a team
- Compassionate, resident‑focused approach to care
- Ability to maintain professionalism, discretion, and respect for resident rights at all times
- Knowledge of clinical procedures, documentation, and care planning
- Ability to develop, implement, and manage programs and policies that support quality care outcomes within the community
- Ability to stand, walk, bend, lift, and move throughout the shift
- Ability to lift up to 25 pounds unassisted and assist with resident transfers using proper equipment
- Visual, auditory, and physical ability to safely perform job duties
- Works in a well‑lit, well‑ventilated community environment with occasional exposure to noise, odors, and interruptions
- May encounter emotionally distressed individuals
- Must be flexible to work weekends, holidays, and extended hours as needed
- Exposure to health and safety risks, including communicable diseases and potential physical hazards
