Working Title:

LICENSED PRACTICAL NURSE -

Performs nursing duties in a County Health Department or outreach clinic offering general clinical services. Performs public health nursing functions according to protocols in specialized areas such as family planning, well child, communicable disease, immunizations, and other chronic diseases. Obtains and records patient health histories, performs nursing duties, triage, administer medication as ordered by physician or practitioner, gives vaccines, performs venipuncture, and performs screening tests, such as vision, hearing, cancer, STD, cardiovascular, diabetes and communicable disease.

Assists in case management activities of family planning and other clinical patients. Refers patients to other appropriate agencies or health professionals. Educate patients as needed. Assists providers as needed.

Performs clinical nursing duties with public health investigations to track and prevent the spread of communicable diseases. Interviews infected individuals to identify and locate exposed partners. Confidentially notify exposed partners of their risk and link them to testing and treatment. Conduct fieldwork and home visits to locate hard-to-reach clients. Assist other health authorities and epidemiologists with communicable disease cases.

Provides immunizations to both children and adults in a clinical and community setting. Ensure that immunizations are maintained at appropriate temperature.

Assist in the review of health records and services. Provides quality improvement activities. Participate in educational training and staff meetings as necessary for professional development and administrative knowledge of program requirements. Prepare all records and reports required to maintain patient information and records of daily work. Participate in record review and chart audits to ensure services and documentation meets expected standards.

Participates in Quality Improvement and Quality Assurance activities as assigned. Attend monthly Clinic Management Team meetings, staff meetings and other mandatory meetings required by supervisor or Health Officer.

All other duties as assigned.

Required Knowledge, Skills, and Abilities:
  • Ability to identify the nature of the problems.
  • Ability to communicate effectively
  • Experience working in a clinic or medical office setting.
Qualifications:

Minimum:

  • 464, F.S. A current state of Florida Practical Nursing License
  • Must have a current driver’s license.
