Pay Range:

• Hourly Rate: $31.94 – $43.92/hr (based on experience)
• Evening Shift Differential: $2.50/hr
• Differentials Stack:
  Working evening shift on weekend = $4.50/hr extra on top of base pay!

Key Responsibilities:

• Perform comprehensive assessment of psychosocial and medical needs of assigned patients
• Develop case management plan of care including clinical, psychosocial, and discharge needs
• Assume leadership role with interdisciplinary team to manage care, level of care, and resource utilization
• Conduct interdisciplinary team meetings for collaboration and planning
• Evaluate admissions for medical necessity using approved criteria
• Perform utilization management reviews and communicate with third party payors
• Act as liaison between physicians, patient/family, hospital staff, and outside agencies
• Make appropriate referrals to third party payer disease and case management programs
• Track and trend barriers to care; make recommendations for process improvement
• Assess patients' post-discharge needs and facilitate services to meet those needs
• Identify patients with high risk complications and make appropriate referrals
• Develop individual plans of care for recurring patients

Key Requirements and Technology

Experience:

• Minimum 3 years of RECENT (within the last year) Case Manager experience in an acute care setting required
• OR: 3 years of recent experience in Med/Surg, Tele, Neuro, ICU, PCU, or ED
• OR:
  Case Manager experience in home health or insurance with 3 years total acute care experience AND at least 1 year of acute care experience within the last 5 years
• Education:
  
  Associate's Degree in Nursing or Diploma in Nursing required
• Education:
  
  Bachelor's Degree in Nursing preferred
• License:
  Current FL RN license required OR compact licensure with active FL RN license within 90 days of hire
• Advanced Practice Registered Nurse (APRN) license acceptable if current and compliant
• Certification in Case Management, Nursing, or Utilization Review preferred
• Regular, punctual, and dependable attendance required
