As we prepare to launch our new Home Health Team in June 2026, we are excited to welcome an experienced Registered Nurse to help shape this growing service line.

• Provide comprehensive nursing assessments and develop individualized Plans of Care in accordance with physician orders.
• Deliver skilled nursing care and interventions based on patient needs and clinical condition.
• Monitor patient progress and adjust care plans as appropriate.
• Administer medications and treatments in accordance with physician orders and organizational policies.
• Provide patient and caregiver education on disease management, medications, safety, and self‑care.
• Coordinate care with physicians, therapists, and interdisciplinary team members.
• Participate in discharge planning, including patient/family education and follow‑up care recommendations.
• Maintain accurate, timely documentation and complete visit notes within required time frames.
• Manage assigned caseload, assist with scheduling, and meet productivity expectations.
• Ensure compliance with infection control standards, patient safety protocols, and regulatory requirements.
• Demonstrate professionalism, compassion, and cultural competence in all patient interactions.
• Perform other duties as assigned.

• Monday–Friday, 8:00 AM – 5:00 PM
• Weekend call rotation required

• Associate or Bachelor’s Degree in Nursing required.
• Active Florida Registered Nurse (RN) license required.
• Current BLS or CPR certification required.
• Minimum of one (1) year of clinical nursing experience required.
• Previous home health, hospice, or related experience preferred.

• Proficiency in Microsoft Office and electronic medical record systems.
• Passion for delivering patient‑centered care in the home setting.

• Valid driver’s license, auto insurance, and reliable transportation.
