RN, Case Manager - Facility team LifePath Hospice

The RN, Case Manager is responsible for assessing and identifying patient/family needs, utilizing the nursing process, coordinating the Plan of Care with the Interdisciplinary Team (IDT), and providing clinical, palliative, and supportive care to the patient/family unit to keep the participant in their home environment as long as possible.

Schedule:

Monday-Friday 8am – 5pm (no on-call or weekend shifts). Sign‑on bonus available. Benefits begin day one.

• Current RN license in the state of employment
• Minimum of one year of nursing experience; hospice or hospital experience preferred
• Certification of completion of Alzheimer’s Disease and Related Dementias Training through the Florida Department of Elder Affairs (for PACE employees)
• Previous experience working with an EMR/EHR system
• Valid driver’s license and automobile insurance (Mobile Driver)
• Reliable transportation to meet visit schedule
• Ability to use equipment with visual and auditory mechanisms
• Effective communication in English (verbal and written)
• Ability to visit participants in their homes for assessments
• Ability to perform essential physical functions (lifting patients or equipment, bending, pushing/pulling, kneeling)
• Active Basic Life Support (BLS) certification from the American Heart Association or Red Cross (some locations may require additional certifications)

• Provides and manages direct care to patients and families as part of the IDT, incorporating psychosocial, spiritual, cultural, physical, and biological components
• Coordinates the individualized Plan of Care, ensuring it reflects evolving patient needs
• Educates patients, families, caregivers, and other health professionals on disease process, prevention, palliative interventions, caregiving, dying process, and safety practices
• Conducts home visits to assess safety, medication compliance, nutritional status, and durable medical equipment usage
• Reports changes in patient condition to appropriate IDT members or other health professionals
• Participates in IDT collaboration meetings and hospice referrals/admissions reviews
• Presents concise oral and written reports to the IDT and respects input from all disciplines
• Communicates accurately and completely with physicians, staff, patients, families, and supervisors; employs positive approaches
• Supervises patient care provided by Community Health Workers and Home Health Aides as requested
• Responds to emergencies (e.g., hurricanes), reporting to designated work locations as required
• Performs other duties as assigned

Perform essential functions such as seeing, hearing, talking, walking, and using hands to handle or feel. Frequently required to stand, sit, reach, lift, bend, and balance. Occasionally required to pull, push, stoop/crouch, kneel, and climb stairs.

Pay range: $61,860.66 – $96,657.28.
