Qualifications:

Must successfully complete the required pre-employment Background and Drug Screenings Must possess a valid Driver's License which meets company standards for approved driver status

Must possess basic computer skills

Must possess a Bachelor’s Degree in the Human Services field

Must have at least one year experience working in the field of intellectual/developmental disabilities

Experience in writing program plans, case notes or other relevant documentation preferred

Essential Functions:

Oversees and coordinates the Program Plan of each individual and ensures it aligns with the individual's needs and desires

Creates positive, collaborative communication with all disciplines and team members

Ensures that Program Plans and documentation are completed in accordance with all regulatory agencies

Assists in monitoring and training of staff and relevant parties to ensure accurate, implementation and documentation of training, support and behavior programs as required by the individuals’ plans

Responsible for assisting individuals to apply for third party benefits such as Medicaid and Medicare Performs all other duties as assigned
