POSITION SUMMARY:
The Case Manager is responsible for developing individualized reentry case management plans, conducting client assessments, documenting daily case notes, and completing monthly case reviews. This role provides comprehensive case management services, including identifying and connecting clients with community resources, facilitating communication between community agencies and treatment providers, coordinating supportive services, and assisting clients in obtaining benefits.
ESSENTIAL JOB FUNCTIONS:
Make referrals and schedule appointments with appropriate agencies, service providers, and case managers.
Build and maintain relationships with property owners and managers to secure locations for outreach activities.
Complete extensive documentation and ensure accuracy.
Prepare and submit quarterly reports as requested.
Assist with data collection, statistical reporting, and the completion of Quality Improvement reports as requested.
Maintain relationships with referral and partner agencies and represent the organization at community meetings as requested.
Coordinate services among multiple agencies to support client needs and service delivery.
Coordinate and facilitate outreach events, distribute Narcan, and connect individuals with available community resources.
Attend scheduled meetings and presentations with referral agencies.
Perform other duties as assigned.

ESSENTIAL
QUALIFICATIONS:

Experience and
Competencies:
Excellent interpersonal and communication skills.
Ability to work independently and as part of a collaborative team.
Compassion, empathy, and a non-judgmental approach towards individuals with substance use disorders.
Understanding to talk to and communicate with clients in an effective and appropriate manner.
Bilingual/bicultural (English/Spanish) is preferred.
Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality.
Professional appearance and demeanor.
Must be culturally/linguistically sensitive to populations served.
Must be able to obtain and maintain level II background clearance.
Valid driver’s license and ability to obtain and maintain West Care’s vehicle insurance is required.
During your tenure with West Care there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that West Care may have to comply with. Should this occur Human Resources or appropriate personnel will inform you.
EDUCATION and/or
EXPERIENCE:
Two (2) years’ experience working in substance abuse prevention, community health promotion, or related fields is highly desirable.
A strong understanding of substance abuse issues, mental health promotion, and evidence-based prevention strategies is essential. Familiarity with theories of behavior change, public health principles, and community organizing approaches is advantageous.
Bachelor’s degree from an accredited university or college with a major in counseling, social work, psychology, nursing, rehabilitation, special education, health education, or related human serviced field is required.

WORKING CONDITIONS:
Work is primarily performed in an outdoor setting
Travel throughout and around the community and outreach is required.
This position may require flexible hours, including evenings and weekends, to accommodate client needs and community events.
