Bilingual Analyst, Case Management Field, North Miami Florida Area

Position Summary

This Analyst Case Management position is with Aetna's Long-Term Services and Supports (LTSS) team and is a field-based position located in North Miami, Miramar, Aventura, Hialeah, or Flagler, FL. The role requires the candidate to travel 50-75% of the time to meet members face‑to‑face.

Schedule:

Monday‑Friday, 8:00 am‑5:00 pm (standard business hours).

The Case Management Coordinator is responsible for telephonic and/or in‑person assessment, planning, implementation and coordination of all case‑management activities with members. The Coordinator evaluates members’ medical needs, facilitates overall wellness, manages a caseload of supportive and medically complex members, and develops proactive interventions to improve short and long‑term outcomes.

The Coordinator determines appropriate services and supports based on members’ health needs, including but not limited to prior authorizations, coordination with PCP and skilled providers, condition‑management information, medication review, and community resources and supports.

• Bi‑lingual in English and Spanish.
• 1+ year of experience in behavioral health or a long‑term care setting.
• Computer proficiency in Microsoft Word, Excel, and Outlook.
• Must reside in North Miami, Pembroke, Miramar, Aventura, Hialeah, or Flagler, FL.
• Must possess reliable transportation and be willing and able to travel up to 50‑75% of the time to meet members face‑to‑face in North Miami, FL. Mileage is reimbursed per company expense reimbursement policy.

• Strong preference for experience in Case Management and Discharge Planning.
• Ability to multitask, prioritize and effectively adapt to a fast‑paced changing environment.
• Effective communication skills, both verbal and written.
• Managed Care experience.

• Bachelor's degree required, preferably in Social Work or related field.
• Master's degree preferred.

Full time

Typical pay range: $21.10 – $40.90 per hour (base hourly rate or base annual salary). Actual base salary will depend on experience, education, geography and other relevant factors. The position is eligible for a CVS Health bonus, commission or short‑term incentive program in addition to the base pay range.
