Duties and responsibilitiesInclude the following. Other duties may be assigned as deemed necessary by management.
- Direct Case Management Services:
Provides direct assistance to identified client including completing an assessment, person-centered service plan, and notes/documentation to facilitate the accomplishment of activities of daily living, personal service plan goals, encouraging community engagement, collaborating and advocating with outside organizations. Demonstrates and coaches the development of skills identified as goals in the service plan. Assist clients in the removal of barriers to self-sufficiency, including applying for and maintaining benefits.
Provide transportation for client to maintain medical or other appointments outside the program. Complete all assigned documentation through communicating information to program staff or/and completing a progress note and uploading required documentation into client’s Electronic Medical Record and other human services system if required by position within 48 hours of service. Minimum client contact is every two weeks. Responds appropriately and in compliance with agency Policies and Procedures to emergent incidents/accidents that develop with client and/or in the environment of care by contacting and reporting to the chain of command and 9-1-1 or the Abuse Hotline, if appropriate.
- Supported Housing Services:
Orient and facilitate the client with entry into housing/shelter. Maintain safety, security, and satisfaction of identified client by monitoring environment of care. Conduct a monthly housing check with emergency drill(s). Responsible for clearing out resident belongs after resident vacates property.
- Outreach:
Meet with professionals from outside agencies to conduct outreach for various programs and collaborate for program success. This may include regular meetings with various groups. Conduct outreach in the community with the homeless population. Enter data into community data bases as appropriate and, if trained, complete SOAR applications.
- Staff Supervision and Evaluation:
Oversee a team of case managers in Lee County regarding the implementation and success of all the above areas. Provides ongoing supervision to program staff. Provide one-on‑one training to new staff and to assist in any problem area or procedural change for the team. Ensure coverage of case load, including documentation, for case managers during any sick, vacation, other leave, or when short staffed.
