Overview
RN Wound Case Manager – Lawton/Ft. Sill Veterans Home, Department of Veterans Affairs. Located at 501 SE Flower Mound Road, Lawton, OK 73501. This full-time position reports to the Director of Nursing and collaborates with the attending provider and interdisciplinary team to coordinate and oversee the facility's wound prevention and wound management program.
Salary: $43.00 per hour.
Responsibilities- Perform comprehensive skin and wound assessments upon admission, readmission, identification of a new wound, significant change in condition, and as clinically indicated. Complete required weekly reassessments and document wound progress, response to treatment, and recommendations in the electronic medical record.
- Assess, stage, measure, document, monitor, and manage pressure injuries, diabetic ulcers, venous ulcers, arterial ulcers, skin tears, surgical wounds, traumatic wounds, moisture-associated skin damage, and other complex skin conditions in accordance with current clinical standards.
- Develop, implement, and evaluate individualized wound treatment plans and resident care plans in collaboration with the attending provider and interdisciplinary team.
- Perform or oversee wound treatments in accordance with provider orders, manufacturer recommendations, facility policy, and evidence-based practice
- Maintain accurate, timely clinical documentation consistent with professional and regulatory standards.
- Promptly communicate new wounds, significant deterioration, lack of expected healing, signs of infection, and other clinically significant changes to the attending provider, Director of Nursing, and appropriate members of the interdisciplinary team.
- Monitor implementation of preventive interventions, including repositioning, pressure redistribution surfaces, offloading devices, moisture management, nutrition and hydration support, mobility, continence management, and other skin integrity measures.
- Collaborate with nursing, dietary, rehabilitation, medical providers, and other interdisciplinary team members to promote wound healing and prevent avoidable skin breakdown.
- Assess nutritional risk factors that may impact wound healing and collaborate with the Registered Dietitian regarding interventions and recommendations.
- Provide education to residents and families regarding wound prevention, treatment plans, and factors that promote healing, as appropriate.
- Serve as the facility's clinical resource for wound prevention and management by providing education, competency validation, and clinical coaching to nursing staff regarding skin assessments, pressure injury prevention, wound treatment techniques, documentation requirements, infection prevention, and evidence-based wound care practices.
- Participate in wound rounds, Quality of Care meetings, infection prevention activities, and Quality Assurance and Performance Improvement (QAPI) initiatives by monitoring wound-related quality indicators, identifying trends, implementing corrective actions, and evaluating outcomes.
- Administer medications and treatments within the scope of licensure.
- Perform other duties as assigned consistent with the position and licensure.
Knowledge of professional nursing principles, evidence-based wound prevention and management, skin integrity assessment, infection prevention, nutrition related to wound healing, and electronic health record documentation. Ability to perform comprehensive clinical assessments, exercise sound clinical judgment, prioritize resident care needs, communicate effectively with residents, families, providers, and the interdisciplinary team, provide staff education, analyze clinical trends, and promote quality improvement, compliance, and resident-centered care.
Demonstrates leadership, professionalism, flexibility, and responsiveness to operational and resident care needs.
Other duties within the Registered Nurse (RN) scope of practice as assigned.

Minimum Qualifications
- Possession of a valid, permanent Oklahoma license as a Registered Nurse in good standing with the Oklahoma Board of Nursing.
