Our SW will work directly with patients and families. This person will be patient, calm and able to manage stress. He or She will have a personable nature, have excellent interpersonal communication skills and must also have superb attention to detail.

Responsibilities may include are but not limited to:

· Evaluate patients (existing, new and prospective) based on their needs, limitations and desires.

· Facilitate care plan meetings and discharge planning

· Completing MDS social work section

· Address patient concerns and goals while maintaining constant communication with patients as well as case managers.

· Build rapport with patient and families.

· Coordinate care with Nursing and rehab departments

· Develop care plans for patients

· Good working knowledge of long-term care regulations including documentation requirements.

· Audits psychology paperwork, hospital discharges, room changes, PASRR and advanced directives.

· Demonstrates judgement of safety and confidential issues

Assumes other responsibilities as assigned by DNS/ADNS
