RN Nurse Educator / Risk Manager

A skilled nursing and rehabilitation facility in Flushing, Queens is seeking an experienced Registered Nurse to serve in a combined Nurse Educator / Risk Manager role. This position leads staff development and clinical education across the nursing department while overseeing the facility's risk management, incident investigation, and survey readiness activities.

Education & Staff Development

• Plan, develop, and deliver new hire orientation for RNs, LPNs, and CNAs
• Conduct annual mandatory in-services and ongoing competency evaluations in accordance with NYSDOH and CMS requirements
• Provide bedside coaching, skills validation, and remediation for clinical staff
• Maintain accurate education records, attendance logs, and competency files for survey review
• Develop training in response to identified trends from incidents, audits, and QAPI findings
• Coordinate CPR/BLS certification, dementia care training, abuse prevention, infection control, and other required curricula
• Serve as a clinical resource for staff on policies, procedures, and evidence-based practice

Risk Management & Quality

• Oversee the facility's risk management program, including identification, reporting, investigation, and mitigation of clinical and operational risk
• Investigate incidents, accidents, falls, medication errors, and allegations of abuse or neglect; complete timely documentation and required NYSDOH/HCS reporting
• Track and trend incident data; present findings to the QAPI committee and leadership
• Support survey readiness through mock surveys, chart audits, and plan of correction development and monitoring
• Partner with the DON, Administrator, and Infection Preventionist on regulatory compliance and corrective action
• Review clinical documentation for accuracy, completeness, and regulatory compliance
• Assist in responding to complaints, grievances, and regulatory inquiries
• Participate in interdisciplinary committees including QAPI, Safety, and Infection Control

• Active and unrestricted New York State RN license (required)
• BSN preferred;
  Associate's degree with relevant experience considered
• Minimum 2 years of nursing experience in a skilled nursing or long-term care setting (required)
• Prior experience in staff education, risk management, quality assurance, or a supervisory role strongly preferred
• Working knowledge of NYSDOH regulations, CMS Conditions of Participation, and the survey process
• Strong writing, investigation, and documentation skills
• Proficiency with electronic health records (Point Click Care  preferred) and Microsoft Office
• Current BLS certification; CPR Instructor certification a plus
• Excellent presentation and interpersonal skills; bilingual (Mandarin, Cantonese, Korean, or Spanish) a plus given the community served

• $100,000 – $105,000 annually, commensurate with experience
