Full Time Physical Therapist Case Manager, Level
2. Anticipated Territory:
Folsom Area. In partnership with other clinicians on the case, you will provide skilled services to evaluate and address the needs of individuals in their homes, so they can live safely and productively in their home environment. Your goals are to restore function to prevent or minimize disability following disease or injury and to help the client reach maximum performance within the limits of their capabilities.
You will evaluate and treat patients using medically prescribed physical therapy programs. You will also supervise 2 Physical Therapy Assistants (PTAs) ensuring they are meeting performance standards.

- Perform an evaluation in the home and develop a care plan; work with PTAs to follow the care plan.
- Help patients with mobility, movement, and regaining independence.
- Create detailed treatment and care plans for each patient.
- Communicate and coordinate with patients’ physicians to ensure a high level of care.
- Evaluate and report patients’ progress to the physician.

- Current unrestricted CA Physical Therapist License
- 1+ year experience supervising PTAs in a home health environment is a plus
- Current BLS/CPR Certification
- Valid CA Driver's License
- Knowledge of local community resources
- Understanding of development, behavior, and cultural factors in health care
- Knowledge of state and federal home care regulatory guidelines (preferred)
- Experience in home health care and with high-acuity patients is a plus
- Experience working with an interdisciplinary team; case management fundamentals are desirable
- Self-directed, empathetic, critical thinker with strong interpersonal skills and organizational ability
