ix Hospice, the Social Worker is an essential member of the patient care team, providing compassionate emotional and practical support to patients and their families in alignment with each individual’s Plan of Care. In this role, the Social Worker meets with patients and their loved ones, collaborates closely with the clinical team, and connects families with appropriate community resources and services.

- Bachelor’s degree in social work from an institution accredited by the Council on Social Work Education.
- Current Social Work licensed in the state(s) of practice
- Five (5) licensure levels:
- Certified Social Worker (CSW)
- Certified Advanced Practice Social Worker (CAPSW)
- Certified Independent Social Worker (CISW)
- Licensed Clinical Social Worker (LCSW)
- Social Work Training Certificate (SWTC)
- Ability to pass background, consumer report, and drug test.
- Conduct psychosocial assessments, identify safety or support needs, and provide resources, referrals, and care planning support for patients and families.
- Provide counseling, crisis intervention, and bereavement support, including follow up for at risk individuals and coordination of bereavement activities.
- Collaborate with the interdisciplinary team, maintain accurate documentation, support cultural and end of life needs, liaise with community agencies, and ensure HIPAA compliance.
- Must have reliable transportation, valid driver’s license, and proof of auto insurance.
- Schedule:
M-F 8:00am to 5:00pm, Full‑time position. - Health, dental, vision, and life insurance.
