Our skilled nursing facility is located in Fond du Lac, Wisconsin, as part of the Eden Senior Care network. We are recruiting positive, happy and reliable Full-time and Part-time Certified Nursing Assistants to join our team.

• Shift Differentials: PMs $1.50/hr, NOCs $1.00/hr.
• Weekends: WE AM $1.00/hr, WE PM $3.50/hr, WE NOC $2.50/hr.
• Tuition Reimbursement/Student Loan Repayment: up to $5,250 per year.

• Provides quality care to Guests in a nourishing environment.
• Provides individualized attention, encouraging each Guest's ability to maintain or attain the highest practical physical, mental, and psychosocial well-being.
• Performs activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and feeding.
• Attends to individual needs of all Guests regarding incontinent care, transferring, ambulation, range of motion, communication, and other needs.
• Maintains each Guest's skin integrity to prevent pressure ulcers, skin tears, and other damage through changing incontinent Guests, turning, repositioning immobile Guests, and applying moisturizers to fragile skin.
• Performs tasks assigned by the floor nurse including checking vital signs, weighing Guests, applying creams/ointments, and collecting specimens.
• Knows individual care plans and supports the care planning process by providing supervisors with specific information and observations of the Guest's needs, preferences, and behavioral changes.
• Assists the Wound Care Nurse as directed.
• Completes medical records documenting care provided and other information in accordance with nursing policies while maintaining strict confidentiality.
• Ensures infection control and standard precaution practices are maintained when providing care and follows safety precautions when performing tasks and using equipment and supplies.
• Reports hazardous conditions, damaged equipment, and supply issues to the supervisor.
• Answers and responds to call lights promptly and courteously.
• Assists with new admissions, re‑admissions, and the transfer of Guests to different rooms within the facility.
• Communicates and interacts effectively and tactfully with Guests, visitors, families, peers, and supervisors.
• Attends and participates in departmental meetings and in services as directed.

• Customer service oriented.
• Certified Nursing Assistant (CNA), or have worked as a Registered Nursing Assistant (NAR), Travel CNA contractor, or similar, and looking for a permanent facility with great benefits.
• Experience in long-term care preferred.
• Able to communicate well in both verbal and written forms.
• Committed to interacting professionally and with care toward residents, families, and other staff.

• Certified or registered as a nursing assistant in the state of Wisconsin.
• Experience in long-term care.
