Travel Nurse (RN) – Home Health / Hospice – 13‑week assignment in Fond du Lac, Wisconsin. Provide end‑of‑life care, collaborate with physicians, and travel within the region. Earn up to $3,300 per week with premium pay and comprehensive benefits.
Key Responsibilities:Provide patient assessments and symptom management for end‑of‑life care, including pain and shortness of breath.
Collaborate with physicians and healthcare teams to develop and implement individualized hospice care plans.
Educate patients and families on hospice benefits, medication management, and end‑of‑life philosophies.
Assess referral patients, determine appropriate hospice level of care, and facilitate admissions within hospital or home settings.
Travel to various hospitals and facilities within the designated region to provide consults, assessments, and hospice services as needed.
Valid RN license in Wisconsin
BLS certification (American Heart Association or equivalent)
EPIC certification or willingness to obtain
Minimum 2 years clinical nursing experience
Valid driver’s license
Auto insurance and reliable transportation
Ability to travel within designated hospital regions
Strong communication skills
Ability to collaborate closely with healthcare teams
Skills:
Hospice or home health experience
Premium pay: weekly direct deposit, 13% above market average, next‑day pay soon

You hold a valid Wisconsin RN license and BLS certification. You have two years clinical nursing experience, preferably hospice. You can travel regionally, hold a driver’s license, communicate effectively
