RN Visiting Hopsice Nurse

Revik Hospice in GLENDORA is seeking a compassionate and dependable Registered Nurse (RN) to provide hospice visits to patients in their homes and other care settings.

This is a per-diem / per-visit position with a flexible schedule. We are looking for an RN who is comfortable working independently while coordinating closely with our hospice clinical team.

• Perform scheduled and PRN hospice nursing visits
• Assess patients and identify changes in condition
• Provide pain and symptom management
• Review medications and provide patient/caregiver education
• Communicate changes in condition to the physician and hospice clinical team
• Coordinate care with the RN Case Manager/DPCS and interdisciplinary team
• Provide compassionate support to patients and families
• Complete accurate and timely clinical documentation

• Current California RN license in good standing
• Current CPR/BLS certification
• Valid driver's license, automobile insurance, and reliable transportation
• Strong nursing assessment and communication skills
• Ability to work independently in the field
• Hospice, home health, palliative care, SNF, or related experience preferred

• Additional compensation may be available for admissions, after-hours, or extended visits, depending on assignment
