Registered Nurse​/RN​/ACTT​/Community Based

We are hiring for a Registered Nurse (RN) in our ACTT (Assertive Community Treatment Team) – a regular, community‑based position. The role involves providing nursing care, crisis stabilization and prevention services to individuals with intellectual, developmental or behavioral health needs.

• Follow supports and services indicated in the recipients’ Crisis Plan, a component of all person‑centered plans.
• Participate as a member of the interdisciplinary team; attend Person‑Centered Plan (PCP) meetings and develop plans as requested.
• Monitor consumers and take appropriate action for side effects and adverse reactions to medications.
• Assist team members in assessing learning needs of consumers and provide teaching as indicated.

• Active RN license.
• Bachelor of Nursing preferred.
• At least one year of nursing experience; experience in behavioral health nursing preferred.
• Valid driver’s license and personal vehicle insurance (required for driving duties).
• Must maintain proficiency in company‑sponsored training, certifications and privileging as needed.
• Complete pre‑employment screening: criminal background check, name checks in registries (OIG exclusions database, Child Abuse Registry, Offenders Against Individuals with Developmental Disabilities), drug testing, education verification and other credentialing.
• Proof of employment history or references (if required).
